Summary
Humana Medicare Advantage updated its coverage policy for lung biopsy and wedge resection procedures (effective 10/01/2026), establishing comprehensive medical necessity criteria across diagnostic and therapeutic indications. The policy requires documentation of clinical indicators, completion of less invasive diagnostic methods when appropriate, preoperative evaluation, and multidisciplinary review for select cases. Billing teams must ensure prior authorization processes align with these updated requirements before the October 2026 effective date.
Action Required
By September 1, 2026: (1) Billing team must update prior authorization system rules to enforce all general requirements (clinical indicators, less invasive method documentation, preoperative evaluation, surgical approach planning, multidisciplinary evaluation consideration) for CPT codes 32096, 32097, 32505, 32607, 32608, and 32666. (2) Create authorization decision trees in the system separating diagnostic lung biopsies (non-cancer and suspected malignancy pathways) from therapeutic wedge resections (NSCLC, neuroendocrine tumors, metastatic lesions). (3) Update prior auth submission forms and provider documentation requirements to capture: symptoms/clinical findings or imaging abnormalities, results of prior non-invasive diagnostic attempts, pulmonary function testing results, surgical approach rationale, and multidisciplinary team input when applicable. (4) Educate thoracic surgery and pulmonology providers on new documentation expectations. (5) Configure system alerts for missing required documentation to prevent claim denials. Failure to implement these changes will result in claims being denied for lack of medical necessity support or incomplete authorization.
