Summary
Independence Blue Cross Medicare Advantage is restricting trigger point injection coverage effective 01/01/2027 by limiting frequency to 3-4 sessions per 12 months, eliminating corticosteroid and other injectates (anesthetic-only now covered), removing image guidance coverage, and restricting covered diagnoses to myofascial pain (M79.12, M79.18 only). Multiple HCPCS codes for injectates and CPT codes for imaging guidance are now classified as not medically necessary.
