Summary
Independence Blue Cross removed three HCPCS codes (J1941, J1938, J3490) related to self-administered furosemide products from coverage under the medical benefit effective 10/01/2026. These drugs will only be eligible for reimbursement through pharmacy benefits, not medical claims. Billing teams must stop submitting these codes to the medical benefit and redirect members to their pharmacy benefit or specialty pharmacy vendors.
Action Required
By September 30, 2026: Billing team must update claims submission system to DENY or REJECT any medical benefit claims containing HCPCS codes J1941, J1938, or J3490 for Independence Blue Cross commercial plans. Update denial reason codes to direct staff to advise members/providers to route these medications through the pharmacy benefit instead. Do NOT submit these codes under the medical benefit effective 10/01/2026. Additionally, do NOT use S9370 (home therapy injection services) when billing administration of self-administered drugs under the medical benefit, as this code encompasses pharmacy services not covered under the medical benefit. Flag any existing prior authorizations for these codes and contact members to transition coverage to pharmacy benefit. Failure to comply will result in medical claim denials and member cost-sharing issues.
