Summary
Independence Blue Cross updated Policy 08.01.33n to add ICD-10 code F64.A (Gender identity disorder, in remission) as medically necessary for leuprolide acetate 3.75mg (J1950) and 7.5mg (J9217), but NOT for Fensolvi (J1951) or Camcevi (J1952). This expands coverage criteria for specific leuprolide formulations for gender identity disorder treatment effective October 1, 2026.
Action Required
By September 15, 2026: Billing team must update coverage rules in billing system to recognize ICD-10 code F64.A as medically necessary for HCPCS codes J1950 and J9217 only. Configure system to deny or require prior authorization if F64.A is submitted with J1951 (Fensolvi) or J1952 (Camcevi). Update claim scrubbing logic to flag mismatched diagnosis-procedure combinations. Train billing staff on the diagnosis-specific code restrictions to prevent claim denials and rework.
