Summary

By 09/15/2026: Billing team must update billing system to reclassify HCPCS codes A2046, A2047, A2048, A2049, A2050, Q4207, Q4223, and Q4243 as experimental/investigational for all bioengineered skin substitute indications; these codes will no longer be covered effective 10/01/2026. Update CPT codes G0681, G0682, G0683, G0684 effective dates to 01/01/2026 in system. Revise clinical documentation templates for lower extremity ulcer cases to capture: (1) wound size ≥1.0 cm², (2) partial- or full-thickness confirmation with clean granular base, (3) infection/necrotic debris status, (4) circulation evidence (ABI ≥0.60 or toe pressure >30 mm Hg), (5) failed response to conservative treatment (≥4 weeks for general wounds, ≥4 weeks for diabetic neuropathic, ≥30 days for venous with compliance documentation), and (6) proof of diabetes diagnosis/medical management (for diabetic ulcers) or ≥3-month venous stasis history (for VLU). Providers must document that bioengineered skin substitute used is on the approved product list for the specific indication. Wound care staff must confirm compliance with appropriate wound therapy requirements (offloading, compression, debridement, infection control). Claims submitted for A2046, A2047, A2048, A2049, A2050, Q4207, Q4223, Q4243 on or after 10/01/2026 will be denied as non-covered experimental services.
