Action Required
By 12/31/2026: Billing team must immediately update billing software and claim submission rules to implement the following changes effective 01/01/2027: (1) Block or deny claims for CPT 77002, 77021, and 76942 when billed with CPT 20552 or 20553 (image guidance no longer covered); (2) Block or deny all claims containing HCPCS codes J0585, J0586, J0587, J0588, J0589, J0702, J1010, J1100, J2919, J3121, J3300, J3301, and J7040 when reported with trigger point injections (injectates other than local anesthetic no longer covered); (3) Implement frequency limit rules to allow maximum 3 TPIs per 12-month rolling period, with system flag for manual review when 4th injection is attempted; (4) Update claim validation to reject TPIs billed with ICD-10 diagnosis codes other than M79.12 or M79.18 (coverage restricted to myofascial pain only); (5) Update provider billing guidelines, encounter form templates, and prior authorization forms to reflect these restrictions; (6) Notify providers in writing of policy changes and required documentation for medical necessity when 4th injection is claimed; (7) Retrain billing staff on new edits and claim denial protocols. Claims submitted without compliance will be denied. Flag all existing standing orders or recurring TPI authorizations for review to ensure compliance with new frequency limits.
