Summary
MassHealth has updated the SACA-2 application form for seniors and people needing long-term care services, removing Third Party Recovery questions and updating various sections including immigration documents, breast and cervical cancer language, and estate recovery information. The March 2025 version is now available and previous versions will still be accepted when applicable.

Immediately: Administrative staff should begin using the updated March 2025 SACA-2 application form available at mass.gov/how-to/apply-for-masshealth-coverage-for-seniors-and-people-of-any-age-who-need-long-term-care. Update any internal references or links to point to the new form. Previous versions may still be processed but using the current version is recommended.
