Summary

Humana Medicare Advantage has updated its coverage policy for diagnostic and therapeutic esophagogastroduodenoscopy (EGD) and esophagoscopy, effective October 1, 2026. The policy clarifies medical necessity criteria by referencing applicable Local Coverage Determinations (LCDs) by geographic jurisdiction, establishes new coverage for preoperative EGD in bariatric surgery workups for both symptomatic and asymptomatic patients, and provides specific guidance on endoscopic ultrasound and optical endomicroscopy coverage. Billing teams must align claims submissions with the jurisdiction-specific LCDs and recognize that preoperative bariatric EGD is now covered nationally.

Action Required

Action needed
By September 30, 2026: (1) Billing team must map all EGD and esophagoscopy claims to the appropriate jurisdiction-specific LCD using MAC assignments (JH/JL for AR/CO/LA/MS/NM/OK/TX/DE/DC/MD/NJ/PA; JJ/JM for AL/GA/TN/NC/SC/VA/WV; JN for FL/PR/US VI; and L34434 for all other jurisdictions J5/J8/J6/JK/J15/JE/JF). (2) Update billing system logic to apply LCD L34434 criteria as the default medical necessity standard for CPT 43235, 43237, 43238, 43239, 43242, 43253, and 43259 in applicable jurisdictions. (3) Implement coverage approval for preoperative EGD (CPT 43235, 43239) in bariatric surgery workups—remove any prior denials for asymptomatic patients and update templates to allow billing without additional clinical justification. (4) Provider education team must communicate that preoperative bariatric EGD is now covered as medically reasonable and necessary nationally. (5) Coding team must verify all optical endomicroscopy claims (CPT 43252) follow LCD L34434 criteria. Failure to apply correct jurisdiction-specific LCDs will result in claim denials or rework. Claims for preoperative bariatric EGD submitted before this date using old denials criteria should be re-reviewed for payment.
