Summary

ForwardHealth is discontinuing the all-inclusive HCPCS code E1035 for adaptive strollers effective July 1, 2026. Providers must now submit base equipment using appropriate HCPCS codes (such as E1161, E1231–E1238) with the UA modifier, and list all accessories as separate line items on prior authorizations and claims. Claims for PAs approved before July 1, 2026 will continue using the old E1035 process.

Action Required

Action needed
By July 1, 2026: Billing team must reconfigure billing software to discontinue use of HCPCS code E1035 for adaptive stroller base equipment and instead map to appropriate base codes (E1161, E1231–E1238) with UA modifier. Update prior authorization submission procedures in billing system to require UA modifier and force individual line items for all accessories. Update claim submission logic to match codes on approved prior authorizations. Train providers and billing staff on new coding requirements. For any adaptive stroller PAs approved before July 1, 2026, continue submitting claims using E1035 under the old all-inclusive policy. Ensure documentation retention per Wisconsin regulations. Failure to implement these changes will result in claim denials for PAs approved on or after July 1, 2026.
