Summary
MassHealth has updated the Application for Health and Dental Coverage and Help Paying Costs (ACA-3) form with minor language changes to step 8 and grammar edits. The March 2026 version is now available and MassHealth will continue to accept previous versions when applicable.
Action Required
No immediate action required for billing operations. MassHealth will accept both the new March 2026 ACA-3 form and previous versions. Staff assisting patients with applications should be aware the updated form is available at mass.gov/lists/masshealth-member-applications but can continue using existing forms.
