Summary
Home State Health Plan implemented seven clinical payment policy changes effective December 20, 2026, affecting behavioral health documentation requirements, burn surgery coverage, fetal surgery criteria, epidural injection frequency limits, therapy services language requirements, genetic testing, and gender-affirming procedures. The most operationally significant change is the Behavioral Health Treatment Documentation Requirements policy (HIM.CP.BH.500/CP.MP.500), which adds nine new documentation elements and modifies existing requirements for mental health claims. Billing teams must immediately review affected policies, update documentation templates and prior authorization workflows, and communicate requirements to providers.
Action Required
By December 20, 2026: (1) Behavioral Health: Update EMR/billing system documentation templates to capture new required elements (I.F. mental health diagnosis certification, I.P. identification of individuals participating in treatment, I.Q. progress documentation, I.R. addenda requirements). Train billing staff and providers on 11 new/revised documentation requirements. Update prior authorization request forms. (2) Burn Surgery: Billing team must remove CPT codes 15271-15278 from burn surgery claim submission rules and update internal coding guidelines to reference CP.MP.185 (Skin and Soft Tissue Substitutes) for skin graft claims. (3) Fetal Surgery: Add CPT 59074 to covered codes list and update criteria to reflect new requirements (hydrops risk assessment, corticosteroid failure documentation). (4) Epidural Injections (CP.MP.164): Update prior authorization templates to require minimum 3-month interval (changed from 2 months) between treatments for relief and functional movement documentation. (5) Therapy Services: Add culturally and linguistically appropriate evaluation note to speech therapy encounter forms for multilingual members. (6) Genetic Testing & Gender Affirming: Review state-specific applicability for marketplace plans and ensure billing rules align with federal 45 CFR § 156.115(d) requirements. Failure to implement behavioral health documentation requirements will result in claim denials. Failure to remove CPT 15271-15278 from burn surgery submissions may trigger improper billing audits.
