Summary

Nebraska Medicaid is implementing coverage for medically necessary inpatient and residential stays in Institutions for Mental Diseases (IMDs) for adults with Serious Mental Illness and children with Serious Emotional Disturbance, effective January 1, 2027. Billing teams must use specific HCPCS codes (H0019, H2018, H2020) with per diem rates or tiered inpatient psychiatric rates, submit claims only after stay completion (not monthly), and enforce a 60-day maximum length of stay with an average target of 30 days.

Action Required

Before Jan 1, 2027
By December 31, 2026: (1) Billing team must configure billing software to accept IMD claims only AFTER the complete duration of stay is finished—disable interim/monthly claim submission for these codes. (2) Update fee schedules in billing system with rates: H0019 (HE modifier) $157.93 per diem, H2018 (HK modifier) $478.97 per diem, H2020 $225.87 per diem for behavioral health claims (CMS-1500), and tiered inpatient psychiatric rates (Day 1-2: $994.30, Day 3-4: $919.55, Day 5-6: $877.34, Day 7+: $835.60) for institutional claims (CMS-1450). (3) Implement system edits to deny/reject claims exceeding 60 consecutive days and flag admissions for ALOS monitoring (target <30 days). (4) Add eligibility validation to exclude QMB, SLMB, QI, QDWI, and emergency-services-only beneficiaries. (5) Train all billing and clinical staff on new claim submission workflow (wait until discharge) and provide documentation requirements. Failure to enforce 60-day limits and post-discharge-only billing will result in claim denials and potential overpayments requiring recovery.
