Summary
Effective January 1, 2027, Anthem Blue Cross and Blue Shield (Virginia) is adding two CPT codes for surgical treatment of hyperhidrosis (32664 and 64818) to their precertification/prior authorization requirements. Providers must obtain prior authorization before rendering these services, or claims will be deemed ineligible for payment.
Action Required
By December 31, 2026: Billing team must update billing software and clinical workflows to require prior authorization for CPT codes 32664 and 64818 (surgical treatment of hyperhidrosis). Providers and billing staff should be notified to submit precertification requests through Availity Essentials (https://Availity.com) or by calling the number on the member's ID card before scheduling or performing these procedures. Failure to obtain prior authorization will result in claim denials and services being deemed ineligible for payment. This applies to all Anthem Virginia commercial plans (HMO, PPO, and EPO).
