Summary

Action Required
By December 31, 2026: Billing team must update the billing system to require prior authorization for CPT codes 32664 and 64818 when billing Anthem Blue Cross and Blue Shield Georgia commercial plans. Update encounter forms and clinical workflows to alert providers that prior authorization is required before scheduling or performing surgical treatment of hyperhidrosis. Verify member eligibility and benefits through Availity Essentials (https://Availity.com) or by calling the member's insurance ID card number before submitting claims. Claims submitted without prior authorization will be denied. Train all staff involved in scheduling and billing on the new requirement.
