Summary

Effective September 1, 2026, Harvard Pilgrim Health Care Commercial (and Point32Health products as of July 1, 2026) will deny all related ancillary and supporting services when a primary service is denied due to lack of prior authorization, untimely authorization, or failure to meet medical necessity criteria. This includes medications supporting the denied procedure, whether billed on the same claim or separately. Specific exceptions apply to radiology professional components, anesthesia, ambulance, pathology professional-only services, and pre/post services outside the date of service.

By September 1, 2026: Billing team must implement workflow changes to identify and prevent denial cascades. (1) Update billing system to flag claims where a primary service lacks prior authorization and automatically review for associated ancillary/supporting services and medications before submission. (2) Implement pre-submission prior authorization verification for all services requiring authorization, particularly those with dependent ancillary services. (3) Train billing staff to recognize services exempt from this denial process (radiology professional component [modifier 26], anesthesia [00100-01999], ambulance [A0021-A0999], pathology professional-only, and pre/post services outside date of service). (4) For Tufts Health Public Plans and Senior Products claims, staff should expect denial code UMD124 with CARC A1 and RARC N19 for supporting services denied due to no authorization. (5) For Maine-contracted providers billing fully insured members with Maine-issued plans, continue current medical necessity review process per Maine statute (24-A MRS §4304); this policy change does not apply. Failure to obtain prior authorization before claim submission will result in denial of the primary service AND all related ancillary/supporting services, significantly reducing reimbursement.
