Summary

Effective November 16, 2026, Priority Health will no longer accept commercial and Medicare claims with a P.O. Box in the Billing Provider address field. Claims must use a street address (or servicing location address) in the designated fields on CMS-1500, UB-04, and 837P claims. Non-compliant claims will be front-end rejected. This requirement already applies to Medicaid claims.

By November 16, 2026: Billing team must audit all billing provider address records in the practice management system and replace any P.O. Boxes with street addresses. Update CMS-1500 Item 33, UB-04 Field 1837, and 837P Loop 2010AA Segment N3*01 to use street addresses only. If no street address is available, use the servicing location address. Train billing staff to verify provider addresses before claim submission. Monitor the Claims tool and Up-Front Rejected Claims search for any rejections related to billing provider address format. Correct and resubmit rejected claims. Failure to comply will result in front-end claim rejections and payment delays.
