Referral Coordination for Certain Institutional Settings for Medical Day Care Services Waiver

REQUIREMENTS: - Effective March 16, 2026: Discharge planning and billing teams must update institutional referral workflows to reflect mandatory CMA assignment for all MDCSW referrals. - SPECIFIC ACTIONS: * Discharge planners must complete revised 3871B forms (selecting 'Home and Community Based Review' dropdown, not Nursing Facility) and submit through Qualitrac system for all institutional referrals to MDC. * Before discharge: Inform all MDCSW-interested patients of the five authorized CMAs serving their residential jurisdiction using the MDC CMA Selection Form; document patient counseling. * Discharge planners must email mdh.mdccmaprogram@maryland.gov with subject line 'Discharge Notice and MDCSW Enrollment Request' containing specific patient identifiers and statement that 3871B has been submitted and counseling completed. * Provide patients with CMA contact list (Table 1: Empowering Minds, Mind Kind, Monumental Health Systems, The Coordinating Center, Total Care Services) including phone numbers and email addresses. * Update discharge protocols and encounter forms to include CMA selection as mandatory step before final discharge. - WHO: Discharge planners, case managers, and billing/administrative staff processing institutional-to-community referrals. - WHERE: In Qualitrac system (3871B forms), institutional discharge workflows, patient education materials, and MDH email notification process. - CONSEQUENCES: Failure to complete revised 3871B workflow, obtain CMA assignment, or notify MDH via required email format will delay or prevent patient enrollment in MDCSW, resulting in service gaps and potential revenue impact from denied claims.
