Signature Requirements for Division of Developmental Disabilities Person-Centered Service Plans

REQUIREMENTS: - Effective immediately (09/10/2026): Billing and clinical teams must update PCSP workflows to ensure all service plans receive documented signature approval from the individual or legally responsible person before claims processing or service implementation. - Update all PCSP templates and encounter forms to include signature lines (handwritten or electronic). - If using electronic signatures: Verify compliance with 13 CSR 65-3.050 and Missouri Medicaid Audit & Compliance (MMAC) Electronic Signature Requirements. Update electronic signature systems accordingly. - Train billing staff to verify signature presence on all initial, annual, and amended PCSPs before submitting claims. Do not process claims for services without documented signatures. - Establish a verification checklist that confirms: (1) Signature is present, (2) Signature is from individual or legally responsible person, (3) All plan modifications (service changes, provider changes, goals, outcomes, rights, legal status, back-up plans) are included in signed version. - For amended PCSPs: Ensure new signatures are obtained for any modifications, not only administrative updates. - Direct questions to TCMTAC@dmh.mo.gov. - Consequence: Claims lacking proper signatures will be denied. Service delivery may not legally proceed without documentation of informed consent.
