(2) Update claims submission templates to require documentation of lesion characteristics (size, differentiation grade, ulceration status, invasion depth) and low lymph node metastasis risk before claim submission. (3) Providers must document all medical necessity criteria in clinical notes and operative reports; include checklist in EMR templates for ESD cases. (4) Front desk/scheduling staff should flag ESD cases for pre-claim review to ensure all required diagnostic documentation (esophageal ultrasound, histopathology results, or imaging per indication) is present before billing. (5) Train coding staff on proper use of unlisted CPT codes (43499, 43999, 45399, 45999) only if C9779 is not applicable or as alternative coding method. Claims submitted without complete medical necessity documentation matching policy criteria will be denied.
