Summary
Independence Blue Cross reissued policy 11.01.01j on 09/02/2026 to communicate their continuing position on otoplasty and non-surgical external ear molding. The policy clarifies coverage criteria distinguishing between medically necessary reconstructive procedures (for trauma, disease, congenital deformities with functional hearing impairment) and cosmetic procedures (which are excluded). Non-surgical ear molding devices are covered only when congenital malformations cause significant functional hearing impairment with diagnostic testing evidence.
Action Required
By 09/02/2026: Billing team must implement the following controls in the billing system: (1) For CPT codes 13151, 13152, 13153, 14060, 14061, and 15260 when linked to ear procedures, require documentation of medical necessity (trauma, disease/tumor with functional hearing impairment, or congenital deformity with functional impairment) before claim submission. Do NOT process cosmetic otoplasty claims. (2) For HCPCS code A9999 (ear molds), require diagnostic auditory testing evidence (ABR) and diagnosis codes Q16.0, Q16.1, Q16.9, Q17.2, or similar congenital malformation codes indicating functional hearing impairment. Reject claims without diagnostic evidence. (3) For trauma-related otoplasty claims (ICD-10 codes S08.111A through S08.121A and M95.10-M95.12), verify procedure is performed within 12 months of trauma event or healing maturation; document pre-trauma/pre-accident restoration intent. (4) Update all encounter templates and prior authorization forms to include checkboxes: 'Reconstructive (medical necessity documented)' vs. 'Cosmetic (NOT covered)'. Providers must select and document appropriately. (5) Train billing staff to identify and deny all otoplasty/ear molding claims lacking required medical necessity documentation or marked as cosmetic. Implement automated edits to flag claims for manual review if documentation is incomplete. Failure to enforce these requirements will result in claim denials and potential recovery actions from the payer.
