Summary
UnitedHealthcare narrowed TMJ disorder coverage to focus on TMJ arthroplasty as the only proven surgical intervention, removing coverage criteria for arthroscopy, discectomy, and reconstruction procedures. Four CPT codes (21060, 21247, 29800, 29804) have been removed from the policy, and the clinical reference standard changed from multiple InterQual® criteria to a single arthroplasty-specific criterion, effective October 1, 2026.
Action Required
By September 30, 2026: Billing team must immediately remove CPT codes 21060, 21247, 29800, and 29804 from all UnitedHealthcare Exchange billing systems and encounter forms—these procedures are no longer covered surgical options for TMJ disorders. Update denial templates and prior authorization workflows to reference only the InterQual® CP: Procedures, Arthroplasty, Temporomandibular Joint (TMJ) criterion for any TMJ surgical requests. Providers must be notified that TMJ arthroscopy, discectomy, and reconstruction procedures will be denied for Exchange members; only TMJ arthroplasty qualifies for surgical consideration. Failure to comply will result in claim denials and potential billing errors. Update medical necessity documentation requirements in EMR templates to align with arthroplasty-only criterion.
