Summary
UnitedHealthcare revised its TMJ disorder treatment policy effective October 1, 2026, narrowing coverage to focus specifically on TMJ arthroplasty (surgery) rather than multiple surgical procedures. Four CPT codes (21060, 21247, 29800, 29804) for other TMJ surgical procedures were removed, and the policy now references updated InterQual criteria for medical necessity. Nonsurgical treatments (physical therapy, injections, splints) remain covered; unproven services (biofeedback, mechanical devices, epigenetic appliances) remain non-covered.
Action Required
By September 30, 2026: Billing team must immediately remove CPT codes 21060, 21247, 29800, and 29804 from all billing systems, templates, and fee schedules for UnitedHealthcare UMR plans. Update any standing orders or provider encounter forms that reference these codes. Beginning October 1, 2026, do not submit claims using these four codes for TMJ procedures—they will be denied. For any pending TMJ arthroplasty prior authorization requests, ensure medical necessity documentation aligns with the updated InterQual® CP: Procedures, Arthroplasty, Temporomandibular Joint (TMJ) criteria. Providers must review the new criteria at uhcprovider.com to understand documentation requirements. For all other TMJ treatments (physical therapy, injections, splints, arthrocentesis), billing processes remain unchanged. Train billing and clinical staff that TMJ arthroscopy, discectomy, and reconstruction procedures are no longer covered under this policy.
