Summary
UnitedHealthcare Surest narrowed TMJ disorder coverage to focus exclusively on arthroplasty as the primary surgical intervention, removing coverage references for arthroscopy, discectomy, and reconstruction procedures. Four specific CPT codes (21060, 21247, 29800, 29804) have been removed from the applicable codes list, and all TMJ surgical medical necessity reviews now require the InterQual CP: Procedures, Arthroplasty, Temporomandibular Joint (TMJ) criteria. Nonsurgical treatments (arthrocentesis, intra-articular injections, trigger point injections, occlusal splints) remain covered.
Action Required
By October 1, 2026: Billing team must remove CPT codes 21060, 21247, 29800, and 29804 from the system's active billing code list for UnitedHealthcare Surest plan TMJ procedures. Update billing software to deny or route for manual review any claims submitted with these codes for TMJ procedures on or after the effective date. Clinical staff and providers must be notified that TMJ arthroscopy, discectomy, and reconstruction are no longer covered surgical options under this policy. For any TMJ arthroplasty procedures, billing team must verify InterQual CP: Procedures, Arthroplasty, Temporomandibular Joint (TMJ) criteria compliance before claim submission. Update encounter forms and prior authorization templates to reflect the narrowed surgical coverage. Claims submitted with removed codes will be denied as non-covered services.
