Update information with BCBSMT, CAQH Provider Data Portal, and National Provider Identifier Registry when changes occur. For removing providers from groups, use only the Demographic Change Form and select 'Remove Provider from Group/Location.' Ensure office staff have current information aligned with what's provided to BCBSMT.
