(1) Map all seven HCPCS codes (Q4116, Q4151, Q4101, A4100, C1763, Q4122, Q4186) to their specific medical necessity indications in the billing system. (2) Configure system to require documentation of one of four covered indications: breast reconstruction, diabetic foot ulcer (DFU), venous leg ulcer (VLU), or burn treatment. (3) For DFU and VLU claims, implement validation to ensure they reference the applicable LCD/LCA criteria (L36377 for J15/J8/JH/JL/JN jurisdictions; state-specific MACs per table on page 2). (4) Update prior authorization templates to request FDA indication documentation and wound size/application duration per policy. (5) Train billing and claims review staff on the distinction between product types (allogeneic, composite, acellular matrices) and their coverage limitations. Claims submitted without documented medical necessity or referencing non-covered indications will be denied. Effective date is 09/01/2026, but advance system configuration is recommended by 08/01/2026.
