Summary

Effective August 1, 2026, new providers joining the network must submit a W-9 form with both legal name and DBA name (if applicable), plus official IRS documentation (SS-4, 147C forms, or other IRS Tax ID assignment notification) as part of their onboarding application. Current in-network providers are not affected unless they have organizational changes like name or TIN changes.

By August 1, 2026: Credentialing team must update new provider onboarding checklist to require W-9 forms with legal name and DBA name plus official IRS documentation (SS-4, 147C forms, or IRS Tax ID notification). Current providers only need to submit updated W-9 if organizational changes occur. Use Demographic Change Form for out-of-network provider name changes.
