Summary
Effective September 1, 2026, Arkansas Medicaid now allows Doulas, Lactation Consultants, and Lactation Counselors to enroll as Medicaid providers and bill for covered perinatal, postpartum, and lactation support services. Billing teams must implement new procedure codes (59409, 59514, 59612, 59620, 59899 for doulas; S9443 with modifiers U1-U6 and HQ for lactation providers) and enforce specific benefit limitations and prior authorization requirements for these services.
Action Required
By September 1, 2026: (1) Billing team must update billing system to recognize and process procedure codes 59409, 59514, 59612, 59620, 59899 (doula services) and S9443 with modifiers U1-U6 and HQ combinations (lactation services). (2) Configure benefit limits in system: enforce 1 delivery code per 9-month period (no extension) for doulas and up to 6 maternal care visits per 13-month period for doulas (with extension available). (3) For lactation codes S9443, ensure first visit cannot be billed as telehealth. (4) Set up prior authorization workflow using Arkansas Foundation for Medical Care (AFMC) process type 126 for extension of benefits requests; require Medical Assistance Remittance (RA) and Status Report with request submitted within 90 days of denial. (5) Update claim submission protocols to include modifier HD for doula codes and appropriate U-modifiers (U1-U6) and HQ modifier for lactation group classes. (6) Train billing staff on provider enrollment requirements: Doulas must hold Arkansas Department of Health certification; Lactation Consultants must hold International Board of Lactation Consultant Examiners certification; Lactation Counselors must hold International Breastfeeding Institute or Academy of Lactation Policy and Practice certification and have a supervising provider; all require NPI. (7) Direct providers to Supportive Maternal Care Provider Manual Sections 210.000 and 220.000 for scope, covered benefits, and billing instructions. Claims submitted without proper benefit tracking or exceeding limits will be denied.
