Summary
Effective July 1, 2026, Providence Health Plan will implement the Optum Emergency Department Claim (EDC) Analyzer to review facility ED claims for Commercial and Medicare members. The analyzer will assess whether billed ED levels (CPT 99283, 99284, 99285) are supported by documented intensity, services, and complexity; claims not supported by documentation may be adjusted or denied. Professional physician ED services are not affected.
Action Required
By June 30, 2026: (1) Billing team must audit current ED facility claim submission practices to ensure CPT codes 99283, 99284, and 99285 are billed in alignment with documented service intensity. (2) Providers must review internal ED leveling guidelines and align with national coding standards. (3) Update ED encounter documentation templates and staff training to emphasize complete and accurate diagnosis and procedure coding on all ED facility claims submitted on UB04 or 8371 forms with discharge status codes 01, 06, or 07. (4) Obtain and review Optum's publicly available EDC Analyzer educational materials to understand how visit intensities are evaluated. (5) Ensure billing team understands that claims not supporting the billed level will be subject to payment adjustment or denial, and that standard reconsideration and appeal rights apply. Failure to align billing with documentation will result in claim denials and payment adjustments.
