Summary
Effective October 1, 2026, Indiana Medicaid is restricting eligibility to only U.S. citizens, nationals, lawful permanent residents (with limited exceptions), Cuban/Haitian entrants, and COFA migrants. All other immigration statuses, including undocumented immigrants, will be denied Presumptive Eligibility and full-coverage Medicaid, though they may still qualify for Emergency Services Only (ESO) coverage. This change impacts eligibility determinations across all Medicaid programs except ESO and requires billing teams to verify immigration status using updated PE application options.
Action Required
By September 30, 2026: (1) Billing team must review and update patient eligibility verification workflows in billing software to align with new immigration status categories in the PE application tool. (2) Front desk and eligibility staff must be trained on the seven qualifying immigration status options (per Table 1) and how to properly document self-attestation. (3) Update patient intake forms and eligibility checklists to capture the specific immigration status categories now recognized. (4) Configure billing system to automatically deny PE claims with reason code 'Applicant Non-Citizen without Qualified Immigration Status' for any statuses not on the approved list. (5) Establish process to route ineligible applicants to ESO Medicaid application pathway. (6) Notify providers that claims for non-qualifying immigration statuses will be denied if billed as full-coverage Medicaid rather than ESO. Failure to implement these changes will result in claim denials and improper patient eligibility determinations.
