Summary
This is a NEW medical policy (effective December 1, 2026) establishing coverage criteria for rhinoplasty procedures. Rhinoplasty is covered only for reconstruction of nasal deformities due to congenital anomalies, tumor resection, or nasal airway obstruction meeting strict criteria (imaging, Cottle maneuver, failed conservative management). Purely cosmetic rhinoplasty remains non-covered. Billing teams must prepare to enforce these criteria retroactively starting December 1, 2026.
Action Required
Before December 1, 2026: Billing and clinical teams must prepare for implementation of Policy SUR12.28. (1) Update billing system validation rules to require prior authorization for CPT 30400, 30410, 30420, 30430, 30435, 30450, 30460, 30462, and 30120 with mandatory documentation of medical necessity. (2) Create or update prior authorization request templates requiring: condition diagnosis, physical exam findings including Cottle maneuver and nasal endoscopy/rhinoscopy results, imaging (CT or equivalent), documentation of conservative management trial (6 weeks minimum) or justification for why it was not indicated, and mechanism of injury documentation if trauma-related. (3) Train providers and front-desk staff that claims for rhinoplasty lacking this documentation will be denied. (4) Implement denial protocol: any rhinoplasty claims for cosmetic purposes only or lacking required documentation post-12/1/2026 will be denied and patient may be billed. (5) Update member communication materials clarifying that cosmetic rhinoplasty is not covered and members will be responsible for costs. Ensure providers obtain patient financial responsibility acknowledgment before rendering cosmetic procedures.
