Summary
Blue Shield of California updated its policy on hematopoietic cell transplantation (HCT) for autoimmune diseases, effective September 1, 2026. Autologous HCT remains investigational for most autoimmune conditions (SLE, rheumatoid arthritis, Type 1 diabetes, CIDP) but is now considered medically necessary for multiple sclerosis and systemic sclerosis under strict clinical criteria. Billing teams must implement prior authorization requirements and update medical necessity documentation standards for these conditions.
Action Required
By August 15, 2026: Billing and clinical teams must implement the following: (1) Create prior authorization checklist in billing system requiring verification of ALL criteria in Section II for MS cases (RRMS or SPMS with inflammatory activity, DMT failure/intolerance, high disability risk, age 18-55, disease duration ≤15 years, EDSS 2.0-6.5, ambulatory status, no significant comorbidities) and Section III for systemic sclerosis (age <60, disease duration ≤5 years, mRSS ≥15, organ involvement documentation, cyclophosphamide exposure <6 months, no active GAVE, no exclusion criteria); (2) Update encounter forms and documentation templates to capture required clinical data (EDSS scores, MRI findings, DMT history, lesion burden, organ involvement measurements); (3) Configure billing software to automatically flag HCT claims for autoimmune conditions OTHER than MS and systemic sclerosis as INVESTIGATIONAL—these require denial or referral to Medical Management for case-by-case review; (4) Train all providers, coders, and prior auth staff on the distinction between covered (MS with specific criteria, systemic sclerosis with specific criteria) and investigational (SLE, rheumatoid arthritis, Type 1 diabetes, CIDP) HCT indications. Consequences: Claims submitted without complete prior authorization documentation or for investigational conditions will be denied. Claims for investigational indications submitted without case review will be denied.
