Summary

Wisconsin correctional facilities must now comply with federal requirements to provide HealthCheck screenings and targeted case management services to Medicaid-enrolled incarcerated youths under 21 or former foster care youths under 26. Facilities can choose to enroll as Medicaid providers for reimbursement (starting April 1, 2026) or submit attestation forms if not enrolling.

For practices serving Wisconsin correctional facilities: Review compliance pathways and determine if your facility will enroll as a Wisconsin Medicaid provider or submit required attestation forms to DHSDMSCAA@dhs.wisconsin.gov. If enrolling, correctional facilities must enroll as billing providers and staff must enroll under specific provider areas (registered nurses, HealthCheck providers, TCM providers). Attestation forms F-03418 or F-03417 are required for facilities choosing not to enroll.
