Summary
Wisconsin ForwardHealth has implemented significant changes to prenatal care coordination (PNCC) provider requirements including new staff qualifications, expanded background check requirements, updated forms, and restrictions on which provider types can offer PNCC services. Home visiting programs are now eligible while HMOs, private case management agencies, and individual nurses are no longer eligible.

By June 1, 2026: PNCC providers in Wisconsin must complete criminal history and caregiver background checks for all staff with member contact, update to new forms F-03188 and F-03184, ensure qualified professionals meet expanded requirements including 2+ years coordinating services for at-risk women, verify care coordinators are 18+ with documented PNCC training, and report staff changes within 10 business days through demographic maintenance tool. Providers must also expand plan of operation documentation and implement new closure notification procedures.
