Effective August 1, 2026, Ambetter Health (Marketplace plans) is implementing stricter billing requirements for Intraoperative Neurophysiologic Monitoring (IONM). Only CPT 95940 with modifier G0453 is covered; CPT 95941 is non-covered. All claims must comply with CMS billing rules, including exclusive single-patient monitoring, maximum 4 units per 60-minute period, appropriate place of service codes (19, 21, 22, or 24), and cannot be billed by surgeon or anesthesia provider.

(1) Configure system to ONLY accept CPT 95940 with modifier G0453 as covered; deny all CPT 95941 claims automatically. (2) Add validation rules requiring exclusive single-patient monitoring documentation. (3) Implement unit-per-time-period limits (maximum 4 units per 60-minute period; total units cannot exceed total monitoring time available). (4) Add place-of-service validation to restrict to codes 19, 21, 22, or 24 only. (5) Configure system to reject claims billed by surgeon or anesthesia provider (include logic to identify billing provider role and deny accordingly). (6) Update claim scrubbing protocols and encounter templates to reflect CMS payability requirements. Providers must stop billing CPT 95941 and non-compliant 95940 claims immediately. Failure to comply will result in automatic claim denials for non-CMS-recognized codes and billing patterns.
