Summary
Effective June 15, 2026, Ambetter from Home State Health (Marketplace product) is implementing stricter IONM billing requirements. Only CMS-recognized payable code 95940 with modifier G0453 is covered; code 95941 is non-covered. Claims must meet five specific criteria including exclusive patient attention, maximum 4 units per 60 minutes, appropriate place of service codes, and exclusion from surgeon/anesthesia billing.
Action Required
By June 15, 2026: Billing team must update all billing software and claim submission rules to enforce the new IONM criteria for Ambetter Marketplace plans. (1) Configure system to ONLY allow billing code 95940 with modifier G0453 for IONM services; flag any 95941 submissions as non-covered and deny automatically. (2) Add validation rules to reject claims that exceed 4 units per 60-minute period or where total units exceed available monitoring time. (3) Create claim scrubbing logic to verify place of service is 19, 21, 22, or 24. (4) Add provider-facing documentation checklist requiring attestation that monitoring was provided with exclusive, undivided attention to one patient and was not billed by surgeon or anesthesia provider. (5) Update provider communication and training materials explaining the change. Failure to implement these controls will result in claim denials from Ambetter, payment delays, and potential compliance issues.
