Summary
Effective immediately, HCPCS codes G0482 and G0483 (definitive drug testing with 15-21 and 22+ drug classes respectively) are no longer reimbursable under this payer's Clinical Policy CP.MP.50. Codes G0480 and G0481 remain covered when medical necessity is documented. Billing teams must immediately stop billing G0482 and G0483 to this payer and update claim submission logic.
Action Required
Effective immediately (6/4/26): Billing team must update billing software to DENY or BLOCK submissions of HCPCS codes G0482 and G0483 to this payer (Ambetter Health). Update claim scrubber rules to redirect these codes or flag for manual review. Providers must use G0480 or G0481 instead when appropriate and document medical necessity in the patient record. Implement internal audit to identify any claims with G0482/G0483 billed to this payer in the past 90 days and resubmit with correct codes.
