Summary
Effective immediately (06/04/26), HCPCS codes G0482 and G0483 for definitive drug testing are no longer reimbursable. Codes G0480 and G0481 remain covered when medical necessity is documented. This impacts all drug screening testing claims using the discontinued codes.
Action Required
Immediately (effective 06/04/26): Billing team must stop billing HCPCS codes G0482 and G0483 for all drug testing claims. Update billing system to deny or reject claims submitted with these codes. Transition all definitive drug testing claims to G0480 or G0481 (which remain covered). Providers must document medical necessity on all drug testing claims per Clinical Policy CP.MP.50. Notify all providers of code discontinuation and update encounter forms/templates to reflect covered alternatives. Claims submitted with G0482 or G0483 after the effective date will be denied.
