Summary

Effective immediately, HCPCS codes G0482 and G0483 (definitive drug testing with 15-21 and 22+ drug classes respectively) are no longer reimbursable under the insurer's clinical policy. Codes G0480 and G0481 remain covered when medical necessity is documented. Billing teams must stop billing G0482 and G0483 and update systems to deny or reject these codes.

Effective immediately (06/04/26): Billing team must (1) Update billing software to deny/reject claims containing HCPCS codes G0482 and G0483 for this payer; (2) Remove G0482 and G0483 from active billing code lists and encounter templates; (3) Route any pending claims with G0482/G0483 for rework or denial processing; (4) Educate providers that only G0480 and G0481 are reimbursable for drug testing going forward, contingent on documented medical necessity; (5) Update internal fee schedules and payer-specific billing rules in the EHR/billing system.
