Summary
Effective immediately, HCPCS codes G0482 and G0483 (definitive drug testing with 15-21 and 22+ drug classes respectively) are no longer reimbursable under Clinical Policy CP.MP.50. Codes G0480 and G0481 remain covered when medical necessity is documented. Billing teams must immediately stop billing G0482 and G0483 and update system rules to prevent claim denials.
Action Required
Effective immediately (06/04/26): Billing team must update billing software to deny/block claims using HCPCS codes G0482 and G0483. Update all encounter templates, standing orders, and provider documentation forms to remove these codes. Contact providers to transition drug testing orders to covered codes G0480 and G0481 (when medical necessity is documented). Audit claims submitted in the past 30 days for G0482/G0483 and prepare for potential denials or claim corrections. Update internal policies and communication to providers explaining the change and directing use of alternative codes. Failure to implement these changes will result in automatic claim denials and revenue loss.
