Summary

Effective immediately, HCPCS codes G0482 and G0483 (definitive drug testing with 15-21 and 22+ drug classes respectively) are no longer reimbursable under the insurer's Clinical Policy CP.MP.50. Codes G0480 and G0481 remain covered when medical necessity is documented. The billing team must immediately stop billing G0482 and G0483 and redirect these claims to G0480/G0481 with appropriate medical necessity documentation.

Effective immediately (06/04/26): Billing team must stop submitting claims for HCPCS codes G0482 and G0483 to this payer. (1) Update billing system to deny or flag any G0482/G0483 claims before submission. (2) For drug-of-abuse testing cases previously billed to G0482/G0483, rebill using codes G0480 or G0481 with supporting medical necessity documentation attached. (3) Notify providers that G0482/G0483 are no longer covered; instruct them to document medical necessity on all drug testing orders going forward. (4) Review and resubmit any pending claims containing G0482/G0483 using the covered alternatives. Failure to implement these changes will result in claim denials and revenue loss.
