Summary
Effective immediately (06/05/26), HCPCS codes G0482 and G0483 for definitive drug testing are no longer reimbursable under Clinical Policy CP.MP.50. Only codes G0480 and G0481 are covered when medical necessity is indicated. Billing teams must stop submitting claims for G0482 and G0483 and update all billing systems and encounter documentation accordingly.
Action Required
Effective immediately (06/05/26): Billing team must (1) Remove G0482 and G0483 from all active billing templates, fee schedules, and billing system configurations; (2) Update billing software to block/reject any claims submitted with G0482 or G0483 codes; (3) Notify all clinical providers that only G0480 and G0481 are reimbursable for drug testing and must be accompanied by medical necessity documentation; (4) Audit any pending claims containing G0482 or G0483 and resubmit with G0480 or G0481 as appropriate; (5) Update encounter forms and EHR templates to reference only G0480 and G0481 for drug testing. Contact your Provider Relations Representative if clarification is needed on which code applies to specific testing scenarios. Failure to comply will result in claim denials and revenue loss.
