Summary
Effective immediately (06/17/26), HCPCS codes G0482 and G0483 for definitive drug testing are no longer reimbursable under the updated Clinical Policy CP.MP.50. Only codes G0480 and G0481 are covered when medical necessity is documented. Billing teams must stop submitting claims for G0482 and G0483 and transition to alternative codes.
Action Required
Effective immediately (06/17/2026): Billing team must update the billing system to deny or block submission of HCPCS codes G0482 and G0483 for all drug testing claims. Configure system to route drug testing claims to G0480 or G0481 only, with medical necessity documentation required. Notify all providers and clinical staff that G0482 and G0483 are no longer billable—claims submitted will be denied. Update internal billing guidelines and encounter templates to reflect covered codes (G0480, G0481 only). Contact Provider Relations Representative if clarification needed on which code applies to specific testing scenarios. Failure to comply will result in claim denials and potential revenue loss.
