Summary
Effective immediately (06/17/26), HCPCS codes G0482 and G0483 for definitive drug testing are no longer reimbursable by this payer. Codes G0480 and G0481 remain covered when medical necessity is documented. Billing teams must immediately stop submitting claims for G0482 and G0483 and redirect to alternative covered codes.
Action Required
Immediately: Billing team must update billing software to deny/block submission of HCPCS codes G0482 and G0483 for this payer. Configure system to reject these codes at claim generation. Notify all providers that G0482 and G0483 are no longer reimbursable effective 06/17/26. Instruct providers to use covered alternatives G0480 or G0481 when medically necessary and properly documented. Review any pending claims containing G0482/G0483 submitted after 06/17/26 and resubmit with appropriate alternative codes.
