Action Required
By October 1, 2026: Billing team must update the prior authorization matrix in the billing system for Ambetter from WellCare of Kentucky plans to reflect the following changes: (1) Remove PA requirement for genetic testing codes 86812, 86813, 86817; (2) Remove PA requirement for home services codes S9211, S9213, S9214; (3) Remove PA requirement for transplant services code 38208; (4) Add PA requirement for pain management nerve block surgery code 64553; (5) Add PA requirement for sinus surgery codes 31297, 31298. Update encounter forms and provider checklists to reflect these new PA triggers. Failure to implement changes by the effective date may result in claims being processed incorrectly (either denied when PA is no longer required or paid when PA should have been obtained).
