(2) Update enrollment forms to include mandatory attestation field confirming training completion with dates and certificate numbers. (3) Create a training documentation tracking system to retain copies of all certificates of completion for each provider. (4) Establish a process to provide supporting training documentation to MCOs upon request. (5) Flag any new CSW provider applications lacking complete training documentation in the credentialing system to prevent enrollment without compliance. Failure to verify training completion before enrollment may result in provider enrollment denial or claims denials upon MCO audit.
