Summary
Beginning July 1, 2025, IHCP (Indiana Medicaid) requires all providers to use the most recent version of the W-9 form dated March 2024 for provider enrollment transactions. Any older versions (October 2018 or earlier) will be returned to providers for corrections.
Action Required
By July 1, 2025: Billing team and credentialing staff must update all provider enrollment processes to use only the March 2024 version of the W-9 form. Download the current form from the IRS website and replace any older versions in enrollment packets. Ensure all pending enrollments use the updated form to avoid delays from returned submissions.
