REQUIREMENT: When submitting CSRs, staff must FIRST select the 'start continuation' button in Cohere Health, THEN upload and complete all required clinical documentation. This applies to all inpatient facilities (hospitals, rehabilitation facilities, long-term acute care, skilled nursing facilities). Update all internal workflows, training materials, and encounter checklists to reflect this sequence. Hold a mandatory team meeting or send written confirmation to all relevant staff before March 31, 2026. Failure to follow this process will result in CSR requests not being flagged in the system, leading to delayed responses, improper escalations, and potential delays in patient care authorization.
