By July 1, 2025: Indiana providers must contact Indiana LCAR-Maximus Help Desk at 833-597-2777 to request nursing facility level of care assessments instead of using ICM agencies or AAAs. Update referral processes and staff training for new LCAR workflow. Hospitals and nursing facilities should continue using Maximus AssessmentPro system but must use full interRAI assessments (no short-form available). By January 1, 2026: Nursing facilities must transition all LOC assessments and PASRR Level I screens to LCAR.
