Ensure the report is submitted together with the Updated Health Equity Plan and Annual Health Equity Assessment Report. No action is required for the documentation described in Exhibit K, Section 10 d.(8)(c), which remains due March 15, 2020. Failure to submit by the corrected deadline may result in contract compliance issues.
