Summary

Oregon Health Plan (OHP) substance use disorder (SUD) residential treatment facilities must now report all member admissions and discharges to the Oregon Health Authority within three business days using secure email and the OHP 7204 form. This ensures proper enrollment coordination and compliance with HIPAA security requirements. Faxing admission/discharge information is prohibited.

Within three business days of any OHP member admission or discharge from SUD residential treatment: (1) Billing/Administrative team must complete the OHP 7204 form with all required member and admission/discharge details; (2) Submit the form via secure email ONLY to ces.dmap@odhsoha.oregon.gov (do NOT fax); (3) Update intake and discharge workflows to trigger this reporting requirement automatically; (4) Train all staff handling SUD admissions/discharges on the new secure email submission process and prohibition on fax transmission; (5) Ensure compliance with HIPAA security requirements when handling personal health information. Failure to report within the three-business-day window may result in improper member enrollment/disenrollment from coordinated care organizations and service disruption.
